Mix together the sage, salt, pepper and garlic, and rub all over the pork.
Place the water in the slow cooker, and place the pork on top. Cook on low for 7-9 hours.
Just over an hour before the pork is finished cooking, make the glaze by mixing together the brown sugar, cornstarch, balsamic vinegar, water and soy sauce in a small saucepan. Bring to a simmer over medium-high heat and stir until mixture thickens, about 3-4 minutes.
Put aside half of the glaze in a bowl for serving and use the remaining glaze to brush over the pork a few times during the last hour of cooking.
Optional: For a more caramelized crust like I have pictured here, remove the pork from the slow cooker once the hour is up and place on a foil lined baking pan. Top with more glaze, and set under high broiler for 1 to 2 minutes, until bubbly and caramelized.)
Serve the pork with remaining glaze on the side.
